Distinguished Budget Presentation Awards Program - Application Form

To apply for GFOA's Distinguished Budget Presentation Award Program, please complete the application below. Applications must be submitted within ninety (90) days of legal adoption of the final operating budget or submission of the proposed operating budget to the governing body, unless an extension has been granted. To complete this form online, applicants will need to have the following information available.

  1. Electronic versions of the budget document, detailed location criteria guide, and your response to comment's from your last submission (if applicable - first time applicants will not include a response to comments)
  2. GFOA log-in and password (you can reset your password from the log-in page).  If you do not log-in with your membership information, non-member rates will apply.

As part of the process to complete this application, you will provide payment through GFOA's e-store for the application.  Click on the appropriate fee to be transfered to GFOA's e-store for payment.  If you do not wish to pay online and would rather pay by check, please check the box below.

After completing payment, please complete the remainder of this form and click submit.  An email confirmation will be sent to the submitter.

After submitting the award application, Please submit a check for the payment amount to GFOA, 203 N. LaSalle Street, Suite 2700, Chicago, IL 60601. GFOA will not process the application or review the budget document until payment in full has been received.

If your organization is awarded the Distinguished Budget Presentation Award, GFOA will provide a certificate. Please enter what you would like on the certificate in addition to the entity name and state. If you leave this field blank, the certificate will only contain the entity name and state. (see example below).

Please verify that your application is complete and ensure that: 

  • 1) You have either provided payment online or plan to send a check to GFOA.
  • 2) Your budget document and all application materials have been uploaded.

GFOA does not evaluate or assess, and makes no guarantees, representations, or warranties regarding, the accuracy or adequacy of the financial information made available by award recipients or the financial solvency or soundness of any government as part of the award programs. Receipt of a GFOA award is not, and may not be inferred or assumed to be, GFOA’s endorsement, evaluation or approval of a recipient’s financial information or practices beyond what is listed in GFOA’s evaluation criteria for each award and governments shall not make or imply GFOA's endorsement for any conditions except those explicitly listed in the evaluation criteria. GFOA award program applicants and recipients, other interested parties, and members of the general public always must rely on their own independent judgment to determine the accuracy, adequacy, or sufficiency of the financial information or budget/policy decisions presented by governments. (regardless of whether they are GFOA award recipients).
